Community Health Care Foundation utilizes the MediCal rate adjusted with the Federal Poverty Guidelines in order to implement a sliding fee discount program so that affordable healthcare can be provided for everybody regardless of financial troubles! A sliding fee discount schedule will be designed to cover reasonable costs of operation and apply discounts on services based on your income. The sliding discount fee program is designed to provide affordable, high quality healthcare for everybody so we can be confident in stress free healthy living!

• Patient eligibility for the Sliding Fee Discount Program, including definitions of income and family size (including what/who is included or excluded) and frequency of re-evaluation of patient eligibility must be disclosed in the application process.

• Documentation and Verification requirements to determine patient eligibility is required through Tax information and pay stubs.
